Each child brought home a mailbag with a communication book and many important papers.
Please fill out the forms and return tomorrow.

Forms to be filled out:
Data Verification form-Please make sure that all of your contact information is correct and anyone that will be picking up your child is included as a contact.
Media Release-Please sign this form so we can take pictures of your child and post them in the classroom and send pictured homes too.
Medical Information– Please make sure that we have any medical or health information that will help use keep your child safe at school
Do Not Ride Form-You only need to return this form if your child will no longer be taking the bus.
*Chickadee-If you fill out this form with a “NO” (or a “YES” if you wish to subscribe) and return it to our classroom, it will allow us to receive a free subscription of this magazine which is very popular in our class.
